Summary

On October 14, 1958, a witness at Wright-Patterson Air Force Base in Ohio observed a square object with a yellow and greenish tint. The object then exploded into pieces. The sighting lasted several seconds, and the witness provided a detailed account to Air Force investigators using the standard UFO report questionnaire.

The witness reported seeing the object during daylight hours. According to the questionnaire responses, the object appeared solid and was bright compared to the background sky. The witness indicated the object was moving when first spotted and noted that it broke apart or exploded during the observation.

Air Force investigators who reviewed the case concluded that the description was characteristic of a meteor. The official evaluation form lists "Probably Balloon" as one of the possible conclusions, though the case file indicates the most likely explanation was a meteor based on the object's behavior and appearance. The eight-page case file is reproduced below as held by the National Archives.
