The cover sheet of this Blue Book case lists the sighting location as Kettering, Ohio, dated October 1962, with an evaluation of "unknown." However, the OCR'd file content does not contain coherent material that specifically describes an October 1962 sighting in Kettering, Ohio. Instead, the 26-page document includes:

- A witness questionnaire form (pages 2-10) with OCR corruption too severe to extract usable details about the witness, object, or location. - A summary notation on page 1 mentioning "two objs w/brightness of 4X Ursa Major flashing about once per second" moving northeast 25 degrees apart, with a note that observers "believed objs to be satellites" and the comment "Possibly a/c since there were two objs." - Multiple unrelated case summaries and sightings from October and November 1962 (Colorado, California, Kent Ohio, Massachusetts, and elsewhere), none of which clearly corresponds to a Kettering, Ohio case. - Administrative routing slips and correspondence unrelated to a specific sighting investigation. - A newspaper clipping about strange objects in western Colorado and a letter from a private citizen to General Curtis LeMay theorizing about radioactive alien objects.
